W-9 Form
The Request for Taxpayer Identification Number and Certification form is a required IRS form for owners of their own businesses and those working as an independent contractor for a business. Your client may request that you fill out and send a W-9 to them so they can accurately prepare your 1099-MISC form and report the payments they make to you at the end of the year.
Company Bank Account Information Form
